The Family Case Management Program helps families with a pregnant
woman, infant or young child to obtain health care services or other
assistance they may need to have a healthy pregnancy and to promote
the infant or child's healthy development.
Our goals are:
- To ensure that pregnant women, infants and children have a physician,
and receive regular physical exams and immunizations
- To help identify and reduce or eliminate barriers to health
care and community services
- To provide health education to eligible clients
- To reduce infant mortality
Eligibility for Service
- Pregnant women and infants in families with incomes at or below
200% of federal poverty level. (Most pregnant women and infants
enrolled in the Women, Infant and Children nutritional program
(WIC) will also be served by Family Case Management)
- High Risk Infants (infants born with medical complications who
spend time in a neonatal intensive care unit)
- Foster children ages 6 years and under
Services Provided
- Health Counseling
- Evaluation of medical and social needs
- Referrals
- Enrollment in KidCare insurance program (if enrolled in the
WIC program)
- High Risk Infant Follow-Up
- DCFS Medical Case Management
Services are provided by five Registered Nurses and one Social
Worker, and are available during office hours, by appointment only.
